How do you plan to pay?

Out-of-pocket? Click NEXT. Wanting to use insurance? FIRST upload a photo of your insurance card (front/back). We must also have a copy of your driver's license (in order to file) of the primary insurance carrier.

NOTE: Mateus is the only provider accepting BCBS PPO plans. All other insurance plans are considered out-of-network and are CASH PAY. A superbill can be provided to you upon request. Check if you understand:

I can only schedule with Mateus if using BCBS. Jill does not accept any insurance. All services will be out-of-pocket. A superbill for self filing for any out-of-network benefits can be provided upon request. WC&NT does not verify your BCBS benefit. A script for self-verification will be provided (co-pays, deductibles, diagnosis coverage, etc). NOTE: We generally find most nutrition services are covered at 100% or a small co-pay. This is solely dependent on your insurance plan, however. WC&NT will file all BCBS claims on your behalf. No need to self-file when using BCBS.
